With a team of 70+ passionate professionals, Stanton Home serves over 20 adults across 8 beautiful homes in the Berkshires (MA & CT). We are expanding and looking for an energetic, empathetic, and driven Case Manager to join our team and help make a difference every day.

What You’ll Do:
As a Case Manager, you will:

  • Build Meaningful Relationships: Work directly with residents, families, and direct care staff to foster growth, support, and well-being.
  • Collaborate: Partner with the Behavior Specialists, Clinical Team, and House Managers to develop and implement medical and behavioral support strategies, ensuring personalized care for each resident.
  • Provide Expertise: Guide the team by organizing and conducting behavior training sessions, ensuring compliance with state standards.
  • Ensure Compliance: Maintain secure and accurate documentation of all resident records, ensuring confidentiality and meeting regulatory requirements.
  • Problem Solve: Actively participate in team meetings and handle crises and behavioral concerns with innovative solutions and a compassionate approach.
  • Support Growth: Help new residents transition smoothly into our homes and programs, ensuring a welcoming, supportive environment.

Qualifications:

  • A degree in a relevant field or equivalent experience with case management is preferred.
  • Strong written and verbal professional communication skills.
  • A valid driver’s license.

Core Responsibilities:

  • Develop professional relationships with residents, families, and team members to provide behavior support.
  • Collaborate with Behavior Specialists and House Managers on intervention plans for behavioral concerns.
  • Attend weekly clinical meetings to discuss resident progress and concerns in collaboration with the clinical team.
  • Full medical care and appointment coordination.
  • Organize and implement training for staff on behavioral responses and intervention strategies.
  • Ensure compliance with state and DDS requirements for documentation and incident reporting.
  • Assist in the admission process for new residents/participants.
  • Conduct monthly visits to residences and provide ongoing support.
