The Person-centered Facilitator assists people and their chosen contributors to actively participate in developing the person’s whole life plan. The Facilitator is responsible for preparing, planning and writing meaningful person-centered plans with quality indicators in mind. The Facilitator monitors progress quarterly and adjusts plans as needed.
Two years of direct experience providing formal or informal support to people with intellectual/developmental disabilities.
Ability to prepare & engage people with intellectual/developmental disabilities in the person-centered planning process
Experience with graphic facilitation
current plan formats (e.g. PATH)
Strong communication skills (verbal, written and face-to-face)
Superior listening skills
Excellent attention to detail and follow-through
The ability to work autonomously and in collaboration with others
High school degree or equivalent
Valid driver’s license, clean driving record and insured vehicle
Enrolled in facilitator training series
Positive, upbeat & open
Supports human rights
Respects cultural values of other
Sense of humor
