Park Lawn is seeking a Self Direction Assistant (SDA) responsible for managing a caseload of participants with intellectual and developmental disabilities in the Home Based Services Program (HBS). The SDA assists participants and families with developing outcomes for their Personal Plan, collaborates with appropriate specialists and consultants on specific case related problems, assists in maintaining eligibility of Medicaid, budgeting/billing and documentation. Our goal is to build and maintain long lasting relationships with each family.
The applicant must be meet the standards of a Qualified Intellectual/Developmental Professional (QIDP). A bachelor’s degree in a Human Services Field and one year experience working with persons with Intellectual/Developmental disabilities is the minimum qualification for this position.
Park Lawn has provided services for persons with Intellectual/developmental disabilities since 1955. Park Lawn is located in the southwest suburbs of Chicago and provides both day program and residential services.
