Job Description

L’Arche is seeking a registered nurse licensed in Washington, D.C. to monitor the health care of the four core people and provide oversight to L’Arche’s Trained Medication Employees and Assistants and all nursing services in our Ontario home. The ideal candidate will have experience working with people who have intellectual disabilities and familiarity with D.C.’s Department on Disability Services (DDS).  In addition, they should be detail oriented, have a passion for community-based care, and be able to provide oversight in a professional and personable manner.

Oversee Healthcare of Each Core Person in Ontario House

  • Conduct all required and requested assessments (HCMPs, Nursing Assessments, Braden Scale etc.) in required timelines.
  • Ensure all medical records, including all medical visit report forms, lab results, referrals, protocols, etc., are organized and maintained.
  • Ensure completion and follow-up of all consults relating to the core person’s health needs, including: labs, diagnostic and preventative testing and procedures, immunizations, nutrition, physical therapy, occupational therapy, and all other referrals.
  • Ensure all electronic records are accessible to assistants and up to date. Coordinate the transition from paper records to electronic records (via Therap) for all medical documentation, as directed.
  • Document nursing discussions, directives, observations, PRN authorizations and all other required findings in online documentation, currently Therap.
  • Complete an in-person monthly assessment for each core person on health issues, changes, etc. Document this assessment in a monthly nursing note. Ensure monthly nursing notes are saved and filed as directed.
  • Ensure that all protocols needed for monitoring health care are reviewed annually by physicians, and accessible to assistants.
  • Consult with assistants, health care providers, pharmacists, and core people family members to address medical and medication questions.
  • Be present to observe DSPs on one routine per week, providing medical feedback to leadership and answering questions that arise.
  • As needed, accompany core person and assistant on medical visits.
  • Attend two team meetings per month, and monthly DC Service meetings.
  • Be on call 24 hours a day, every other week, to provide PRN authorizations and nursing coverage for both DC houses, including provide cross-coverage for nursing tasks for Euclid House, as needed.

Trained Medication Employee Program

  • Comply with all DDS and BON requirements for oversight of Trained Medication Employees (TMEs), including but not limited to:
  • Medication Management
  • Oversee monthly changeover of medications in Ontario.
  • Sign off on MARs/POS monthly, and new MAR/POS’s during the month.
  • Check all MARs weekly to ensure that medications are properly administered and documented.
  • Oversee all TME activity
  • Ensure that a nurse administers first dose of every new medication.
  • Be observed (twice), and observe (four times), new TMEs prior to their giving medications.
  • Ensure that all eligible assistants have appropriate and current certification as TMEs
  • Complete quarterly (or as required) TME reviews.
  • Review medication errors with TMEs and counsel/retrain as necessary.
  • Develop and conduct TME continuing education program.
  • Implement policies and procedures to minimize medication errors, in coordination with Service Team Leadership.

Training for Assistants

  • Train on delegated nursing tasks, medical training, Health Care Management Plans and Phase II training for all new assistants. Conduct annual and as needed retraining for the Ontario team.
  • Oversee development and implementation of falls protocols, mealtime protocols, seizure protocols and other protocols, as needed.
  • Provide counseling and corrective action to resolve any gaps in performance related to health issues or medical services, in collaboration with Service Team Leadership.
  • Develop and conduct continuing education training for all assistants at least twice/year.
    Professional Training and Quality Assurance Coordination
  • Ensure compliance with all DDS policies and regulations regarding nursing care and medical, including Health and Wellness Standards. Attend required DDS trainings and implement new requirements with service team.
  • Collaborate with leaders to determine staffing needs/changes.
  • Coordinate with Quality Improvement team to develop policies, procedures and practices that improve quality of medical care for core people
  • Actively monitor and respond to quality assurance issues on “MCIS” dashboard that relate to nursing care or medical oversight.
  • Attend local nursing groups, including familiarity with DDS Health and Wellness Unit; meet with representatives as necessary.
  • Ensure compliance with all DC Board of Nursing Regulations, the DC Department of Health regulations, DDS Health and Wellness regulations, and any other policies or procedures applicable to L’Arche.
  • Become Master Trainer for TMEs.
