SUMMARY: This position is responsible to provide Strength based, solution focused, supportive therapeutic services to the individuals served. The Case Manager engages individual in development of their Individual Program Plan to support them in achieving their habilitative goals.


Complete Assessments upon admission and annually (more often as needed).
Facilitate Quarterly Person-Centered Planning Meetings for each individual on their caseload.
Develop Individual Program Plan (treatment plan) with the individual served.
Complete Daily Documentation in Therap
Participate in Screening of individuals referred to the program.
Coordinate and link individuals served to appropriate community resources.
Assist individuals in managing their personal finances moving toward independence in financial management.
Assist individuals with gaining, restoring, improving, and/or maintaining daily independent living, social/leisure, and personal hygiene skills.
Ensure all basic necessities are met (clothing, bedding, personal property ect…).
Attend any court appointments with the individual.
In conjunction with attending Therapist, develop and implement Therapeutic Schedules to meet the needs of the individuals in the program.
Provide guidance and supervision to Direct Care Staff.
On Call for behavioral and administrative issues as they arise.
Performs other related duties as assigned by management.


Indirectly supervises Direct Support Providers employees within the Residential department(s).
Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws.
Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.


Bachelor’s Degree (BA) from four-year college or university in a human service related field with at least 1 year experience working with individuals with Intellectual and Developmental Disabilities.
Computer skills required: Microsoft Office Suite
Other skills required:
Ability to Pass CPR, First Aid and Pro Act Restraint training
Ability to maintain strict confidentiality (HIPPA)

Occasionally required to stand.
Occasionally required to walk.
Occasionally required to sit.
Frequently required to utilize hand and finger dexterity.
Occasionally required to climb, balance, bend, stoop, kneel or crawl.
Frequently required to talk or hear.
Occasionally exposed to bloodborne and airborne pathogens or infectious materials.
While performing the duties of this job, the noise level in the work environment is usually moderate.
The employee must occasionally lift and/or move up to 50 pounds.
Specific vision abilities required by this job include: (close vision; distance vision; peripheral vision; depth perception and ability to adjust focus.
Ability to perform CPR and Pro Act Restraint techniques.
