Serra Center is seeking a Program Supervisor / QIDP (Qualified Intellectual Disabilities Professional) to oversee daily operations of our 3 ICF-DD/H group homes licensed by the CDPH. The role involves managing, coordinating, and monitoring each client’s active treatment plan.

Qualified candidates must possess experience with program management, working in an exempt, leadership/management role, and be willing to serve one week of 24-7 on-call rotation once a month.

POSITION REQUIREMENTS:

A Bachelor’s degree in a related field like human services, social work, psychology or nursing is required.
At least 1 year experience working directly with individuals with intellectual or developmental disabilities.

EXAMPLES OF JOB DUTIES / RESPONSIBILITIES: (not an inclusive list)

Individual Program Planning: Develop, implement, and review individualized program plans that meet the unique needs of each client.

Case Management: Manage a caseload of up to 18 clients, assessing their needs, and coordinating services.

Direct Support Staff Supervision: Supervise and train direct support staff who work directly with individuals.

Family Coordination: Work with families to ensure their involvement and support in the care plan.

Monitoring and Documentation: Track progress, document interventions, and make adjustments to the plan as needed.

Compliance: Ensure homes are operating in accordance with Title 22 and CDPH Licensing requirements.
