- Education and Experience
- Bachelor’s degree in related field required.
- Minimum of 2 year’s experience working in human services. Experience coordinating resources and individual planning processes and working with funding agencies preferred.
- Key Responsibilities
- Consistently report to work on time as scheduled, prepared to perform assigned duties and responsibilities during entire scheduled work period/shift.
- Meet with each individual and family annually and as requested, and coordinate with other staff that know the individual well, prior to PCP meeting to assess the individual’s goals and desires for living, working and leisure for the future.
- Prepare DDA and Benedictine required assessments and reports in advance of PCP meeting based upon the discussion which includes background, strengths/achievements, needs/barriers, likes/dislikes and recommendations and implementation strategies for identified goals.
- Attend each individual’s PCP meeting to present report to team members chosen to help him/her achieve future success.
- Work with individual, day and residential supervisors, and staff to achieve goals established in the PCP including providing individualized staff training as requested to understand and implement goals.
- Visit homes and job sites, as appropriate, to monitor progress, resolve problems and maintain effective relationships as scheduled and needed, but at a minimum quarterly.
- Submit quarterly updates on progress toward goals according to the PCP.
- Review goal documentation maintained by direct support staff on a regular basis.
- Review General Event Reporting (GERs) as submitted. If needed, make referrals, incorporate and follow behavior protocol for each individual as it fits into his/her plan.
- Advocate with and for the individual to ensure that they are working toward the goals that they choose and achieve the quality of life they desire, balanced with ensuring best possible health and personal safety.
- Collaborate and serve as primary Benedictine correspondent with DDA Coordinators of Community Supports regarding services, supports and funding.
- Confirm accuracy of service funding and approve plans in LTSS within established time frames.

- Hours of work
- Monday thru Friday; primarily day shift, but hours may vary based on individual needs; some on-call shifts required.
- Overtime expectations
- This is an exempt level position.
- Work environment
- Ridgely Campus and/or Group Home Setting.
- Travel requirements
- Provide transportation utilizing Benedictine fleet vehicles.
On-call responsibilities
- Rotate weekend on-call requirements and some on-call requirements during normal residential hours may be required.
