Under direction of a Program Manager, provide overall service coordination to assigned clients; participate in the assessment of the needs of persons with developmental disabilities; act as team leader and facilitate the development, implementation and coordination of an Individual Program Plan (IPP)/Individual Family Service Plan (IFSP); assist in locating and securing services and supports; act as an advocate; evaluate and monitor the quality and effectiveness of services.
EDUCATION/EXPERIENCE
Bachelor’s Degree preferably in a Human Services field and 1 year related experience which must be in the field of developmental disabilities for non-Human Services majors; or have a Master’s Degree in Social Work, Psychology, Special Education, or a related field.
STARTING SALARY
BA/BS Entry Level $25.04 to $27.54 an hour/Master’s Entry Level $28.80 to $31.67 an hour
