Care Coordination consists of services and staffing provided to each individual by a single, identified person. The Care Coordinator is responsible for coordinating the delivery of services included on each consumer’s Individual Plan of Care (IPC), monitoring delivery of waiver and generic services, integrating various aspects of services delivered under the waiver, and recording each individual’s progress. The Care Coordinator will ensure that services are justified and implemented in accordance with the IPC and the Person Directed Plan.

Previous exposure to an HCS environment is preferred

Must maintain a valid driver’s license and driving record acceptable to Kenmar Residential HCS Services Inc. insurance requirements
Essential Functions and Responsibilities:
Oversees and ensure all program areas are monitored and addressed routinely
Completes the development and implementation of each person’s Individual Plan of Care and Implementation Plan annually and completes revisions as needed
Conducts face-to-face visits with assigned service recipients at least quarterly
Completes a regular review of each assigned service recipient’s progress towards outcomes as well as service utilization
Participates in regularly scheduled meetings
Participates in scheduled advisory committee meetings as assigned
Reviews and approves consumer expenses monthly or as needed
Coordinates Medicaid services via initial application, annual or semi-annual review for people for whom Kenmar is assigned Representative Payee; and, provide assistance as needed for those Kenmar is not Representative Payee.
Coordinates Social Security benefits for people for whom Kenmar is assigned Representative Payee
Coordinates continuous integration with other intra/inter agency staff, consultants, families and guardians
Completes renewal and revision of ICAP for each Consumer served
Completes renewal and revision of the IDRC/Level of Care for each Consumer served
Coordinates and monitors the delivery of waiver and non-waiver services, including review of service delivery documentation
Maintains the record of individual service information for each Consumer served
Completes entry into CARE of IPC’s, IDRC’s, suspensions, discharges, location changes, address and guardianship updates
Obtains information from waiver and non-waiver service providers as needed
Makes referrals for consumers as needed in accordance with service plans and individual needs
Provides crisis intervention as needed with minimal supervision
Provides person-specific training of residential staff and contractors to ensure adequate service delivery
Maintains positive relationship with other Kenmar staff and contractors to ensure continuity of care
Participates in on-call duties
Maintains working cell phone, either personal (via stipend) or company provided.
Maintains compliance with HCS principles, billing guidelines and service definitions
Intervenes verbally and physically when necessary to prevent injury to consumers and employees and to provide appropriate first aid care to individuals and employees who are injured or ill.
Ensures physical intervention is provided quickly and appropriately utilizing appropriate techniques (SAMA)
Ensures First Aid/CPR (cardiopulmonary resuscitation)/ Universal Precautions are utilized when necessary
Performs other duties as assigned
Education and/or experience
Bachelor’s Degree in a health and human services field or a high school diploma/ GED with at least 4 years of verifiable experience working in the field of intellectual and developmental disabilities.
