JOB SUMMARY: The AFC Care Manager will plan, coordinate and monitor the member’s AFC services. S/he will provide psychosocial and counseling assistance for the AFC program. The Care Manager may function as the AFC Program Director. Care management includes assessment and ongoing monitoring of care provided by the AFC caregiver. Initial assessments, scheduled visits, communication with the RN nurse, documentation and training and supervision of care givers. Position also includes periodic reviews of logs, procedures and regular site visits.
Bachelor’s degree and two years clinical experience direct care role with elderly/adults with disabilities, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years recent experience working with elderly/ adults with disabilities. Ability to work within an interdisciplinary team environment. Current valid driver’s license and car
OTHER QUALIFICATIONS: Must have strong oral and written communication skills with consumers, program staff, families, therapists and other service providers/monitors. Capacity to learn/perform basic sign language, CPR/First Aid and other augmentative communication systems essential. Computer literate.
SALARY RANGE: Mid $30,000 depending on education and experience. Full benefit package.
